2. We build your care plan
Home health is physician-ordered, so we coordinate directly with your doctor to create a personalized plan. If you don’t have an order yet, we’ll help you navigate that step.
3. Care comes home
A Utah-licensed nurse or therapist begins visits on your schedule, in your home. Your plan is reviewed and adjusted as your needs change.
What to expect on the first visit
Your first visit is an assessment: a nurse reviews your health, medications, home environment, and goals, then finalizes the plan of care with you and your physician. It’s relaxed, thorough, and built around you.
